CDC on Homemade Cloth Face Coverings Effective 3/20/20 To enhance everyone’s safety and ensure appropriate social distancing we’re modifying the Walgreens Pharmacy shuttle program in the following ways: The shuttle will run by appointment only instead of at regularly scheduled times No more than 2 students will be allowed per shuttle All riders will be required to satisfy Northern Berkshire EMS COVID-19 screening protocols before being allowed to take the shuttle The pharmacy shuttle will depart from and return to Hopkins Hall Drive (outside the entrance for Campus Safety & Security). Students should call (413) 597-4545 to request a ride. Be prepared to provide your name and call-back number. These additional precautions are likely to increase wait times to get the pharmacy, so please only use this service if it is imperative you pick up a prescription. Please plan appropriately and allot as much time as possible when you call to make an appointment. Medical Record Requests and Required Forms Forms Release of Medical Information Form This form is not required for newly entering students . Should you wish to request a copy of your student health record, please follow the instructions on the form and return to the Health Center by email to [email protected] or via fax @ 413.597.2982. Your request will be processed during the academic year when the Student Health Center is open. Please allow up to two weeks for processing of your request. RELEASE OF MEDICAL INFORMATION form Required Health Forms for First Year Students and Students new to Williams College All first-year and transfer students are required to complete and return Williams health forms. To do so, a physical exam is required within 12 months of July 1 . Students planning to participate in intercollegiate sports must have a physical within the 6-month period preceding their sport season: August 30 for Fall sports, November 1 for Winter and Spring sports The complete instructions and all REQUIRED forms for first-year and transfer students are accessed and submitted through the Williams College Health Services Patient Portal . You may submit any questions you may have regarding the required health forms or the deadline via email to health​@williams​.edu , using “Health Forms” as the subject line.
